Are you interested in supporting people with cancer to prepare for their surgery? An exciting opportunity has arisen for two Clinical Support Workers (Higher level) to join our two-year pilot cancer prehabilitation programme that is being run in association with Macmillan Cancer.
The programme is a multidisciplinary, personalised prehabilitation package of care for patients undergoing major cancer surgery at LTHT. The Shape Up 4 Cancer Surgery personalised prehabilitation programme is being delivered as a partnership between LTHT, Leeds City Council Active Leeds, Leeds Beckett University, University of Leeds and Macmillan Cancer Support. It will target patients on an expanding number of cancer pathways including: Urology (Cystectomy), Upper-Gastrointestinal, Pancreas, Colorectal, Thoracic and Maxillofacial. As part of this team, you will be providing high-quality care under the supervision of a qualified nurse who is the programme lead. You will be required to assist with the planning, implementation and evaluation of individual programmes of care for patients. You will maintain regular contact with the patients who are undergoing intervention, requiring excellent telephone communication skills. You will be actively involved in the development, implementation and audit of patient care practices as part of Clinical Governance. Main Duties of the Job
We are looking for motivated and enthusiastic clinical support workers (High level) to join the Shape Up 4 Cancer Surgery programme within LTHT. The role will include direct clinical contact with clinicians and patients as well as an opportunity to be involved in data collection and audit work. A close working relationship with the LTHT pre-assessment team and various Clinical Nurse specialists will need to be developed. The successful candidates will report to the Shape Up 4 Cancer Surgery Programme Co-ordinator and will participate in all aspects of the Shape Up 4 Cancer Surgery project. This includes ensuring the delivery of high-quality care for patients in an outpatient clinic setting, and remotely via telephone whilst waiting for their cancer surgery. The post-holders will be actively involved in providing direct and indirect compassionate care for patients in support of and supervised by a registered practitioner. The post-holders will assist in the overall implementation of care as part of the multi-professional team. The post-holders will be skilled in taking appropriate action(s) to achieve the planned outcome and contribute to the maintenance of an environment for effective care. Candidates will have NVQ level 3 or equivalent. Prior experience of prehabilitation is not required but knowledge and experience of pre-assessment or the operating theatre environment is desirable. All essential and required training will be provided.

Job Responsibilities
The post holders will assist in the overall implementation and evaluation of care as part of the multi-professional team. The post holders will be skilled in taking appropriate action(s) and acting on their own initiative to achieve the planned outcome.
The post holders will be involved in providing direct and indirect patient care independently but with support available from a supervisor within the clinical area. In doing so, they will support the assessment of service users and implementation of programmes of care delegated to them by the appropriate staff member ensuring that they practice within their sphere of competence and knowledge.

* Following training and assessment, competently undertake clinical activities relevant to the post and clinical area as agreed.
* Demonstrates developed physical skills obtained through practice and training e.g. accuracy, dexterity and coordination to complete routine tests within agreed pathways of care, manipulate equipment.
* Exchanges factual information with service users using persuasion, reassurance, tact and empathy and addressing barriers to understanding to engage the individual in their care and gain valid consent; ensure that all discussions are recorded in an accurate and timely manner in the patient record.
* Engages and actively involves the individual and, if appropriate, their family/carer, in their assessment, programme of treatment and/or care, if directed to do so by the responsible professional, by using person-centred techniques to promote a culture of self-care, involvement and empowerment.
* Approaches each individual with care, compassion and sensitivity ensuring that these values are reflected in all aspects of the role.
* Be responsible for delegated administration tasks and management of resources within the clinical area e.g. stock control, basic equipment cleaning, specific clinical tasks to support the smooth running of a clinic.
* Communicate with colleagues and members of the multi-disciplinary team clearly and effectively in written, verbal and electronic format (this may include accessing and updating patient records on central databases or electronic systems).
* To collate data for departmental audits, and act on findings under guidance.
* Evaluate and review the effect of any interventions and the patient's agreed plan of care and report outcomes of interventions to the registered practitioner.

Qualifications
Essential
* NVQ 3 in Health and Social Care.
* Diploma in Health Level 3, Diploma in Health and Social Care Level 3 or equivalent health care related qualification.
* The Care Certificate or in the absence of this the post holder must successfully attain the Care Certificate within the first 3 months of employment.
* Commitment to undergo training and education as required.
* Significant experience within a Band 3 CSW post with an obligation to complete the relevant qualification within the first 12 months of employment.
Desirable
* Basic skills Maths and English Level 2 (equivalent of GCSE 4-9 (A*-C)).
