Supportive Outreach and Engagement Worker (Alcohol Care Teams)
A new opportunity has arisen to join the MFT Alcohol Care Team (ACT). This is an exciting time to join the team, as it expands and develops within the hospitals and the wider trust.
The Supportive Outreach and Engagement Worker position has been created to support individuals accessing secondary emergency care services, providing a wider, holistic and multi-agency approach to case management.
Facilitating integrated alcohol care between secondary, primary and community services for acute alcohol frequent hospital attendees.
This role will be primarily community-based; however, you may at times be requested to work at one of the 3 Alcohol Care Team locations: Manchester Royal Infirmary, Wythenshawe Hospital, or North Manchester General Hospital.
Main duties of the job
The focus of the role would be working with high-intensity users of the trust who present to the Emergency Departments or are admitted in relation to alcohol misuse. This includes ensuring effective implementation of care pathways and being the interface between community substance misuse services and the Alcohol Care Teams. The post holder will attempt to re-engage individuals who have dropped out of treatment or detached from services.

Education/qualifications
* Registered Nurse - NMC
* 1st Level degree in related area or working towards
* English and Maths GCSE or equivalent
* Evidence of continuing personal development/revalidation and post-registration training in nursing care assessment and treatment with substance misuse clients
* Experience of working with a range of organisations and agencies to develop effective working relationships
* Hold a current driver’s license and car user
* Registered independent non-medical prescriber
* Qualification in CBT
* Completed clinical examination skills module
* Leadership course
* Motivational Interviewing training
* ENB 998 Teaching and assessing in clinical practice (or equivalent)
* Evidence of health Service Management
Experience
* Experience of working independently
* Experience of working with individuals with Alcohol use Disorders/substance misuse
* Experience of working with hard-to-reach and difficult-to-engage individuals
* Experience of assessing service users’ needs and working in partnership with them and other services to set and monitor goals
