The team is multi-disciplinary and provides specialist health support to both individuals, and their wider support networks. The team work agilely and covers the Kirklees locality. Main duties of the job Will carry out to a high standard (Under the supervision/direction of the registered Nurse) specific tasks/planned programmes of care with a designated group of service users and be able to assess, problem solve or report, in a timely and proactive manner, when these standards have not been met.
