The Dementia Rapid Response Team invites your application!
Based in Derby City & South Derbyshire, our service promotes a person-centered, non-pharmacological approach to support older adults diagnosed with dementia, especially during crises in their mental health.
We serve the Derby City and South Derbyshire area.
The goal of the service is to help maintain the independence and dignity of those living with dementia, aiming to prevent hospital admissions where possible.
Additionally, the DRRT supports early hospital discharge, working closely with carers and relatives.
The service operates year-round, Monday to Friday from 8 am to 8 pm, and weekends and bank holidays from 9 am to 5 pm.
Main duties of the job
The post holder will:
1. Employ a wide range of skills, developing expertise in managing complex cases as part of a multidisciplinary team including Occupational Therapy, Physiotherapy, Nurses, Dementia Support Workers, Psychology, and a Consultant Psychiatrist.
2. Be clinically responsible for supporting the delivery of a flexible, responsive community service.
3. Travel independently across the catchment area, and occasionally outside it, as required.
4. Work a shift pattern that supports the service’s operation, which runs 7 days a week.
5. Undertake the Duty role: triaging referrals, allocating visits, preparing and delivering handovers, and ensuring colleagues’ safety and well-being.
6. Collaborate with a variety of professionals and agencies as part of the MDT.
7. Complete initial assessments of patients.
8. Apply knowledge of the Mental Capacity Act and Mental Health Act when necessary.
9. Assess risks and develop safety plans.
10. Formulate person-centered care plans emphasizing non-pharmacological interventions.
11. Participate in inpatient ward in-reach and attend 117 meetings.
12. Visit patients at home and in 24-hour care settings.
13. Use the EPR system for accurate documentation of assessments and treatments.

Person specification
Experience
* Recent, relevant post-qualification mental health nursing experience
* Community nursing experience
* Experience working with older adults and those with dementia
* Up-to-date Personal Development Plan/Professional Portfolio
Qualifications
* Supervision trained
* Evidence of leadership skills such as motivation, facilitation, mentorship, etc.
Skills
* Knowledge of MHA, Capacity Act, and legal frameworks
