The Community Mental Health Team (CMHT) is a generic team integrated with Croydon Social Care in working with all adults experiencing severe and enduring mental illness.
We are looking for an enthusiastic discharge coordinator that will grow and develop with the locality teams from Nursing, Social work, and OT professional background.
This advert is for vacancies in CENTRAL LOCALITY TEAM.
Main duties of the job
The Post holder will be a CPN, OT or social worker working as a discharge coordinator in Croydon's newly formed locality teams working directly with care coordinators to identify patients whose care needs are best met in another appropriate service.
The post holder will work closely with Care coordinators in identifying any barriers to discharge upon acceptance into the team implementing community care process model, CPA/review meetings. They will work 'hand in hand' with their colleagues in the team, other community teams in and out of the trust, primary care colleagues and the patient's family/carers to facilitate discharge. They will ensure that the estimated discharge date as identified during reviews is progressed where possible and feasible.

Job responsibilities
Clinical:
To take the lead in the discharge planning for patients that no longer require intensive input from the CMHT or are required to be transferred to another team for varied reasons. To ensure comprehensive and accurate patient information collection, from initial referral, through their stay with the team and on to discharge. To use this information to complete transfer and discharge CQUINNs forms accurately to GPs or other CMHTs, and other teams within the CMHT. To support discharge from care coordination by working through any barriers to discharge at the outset of the acceptance into the team. To deliver a range of effective, service user-centred interventions aimed at crisis resolution and promoting recovery. Supporting service users to adopt recovery strategies that promote their wellbeing, healthy lifestyle, independence and self-care. To undertake thorough risk assessment of all service users in line with policy standards to manage risk safely and effectively to enable effective and safe discharge. To ensure that discharge plan is carefully and meaningfully planned within the multidisciplinary team, facilitated effect delivered to a high standard and evaluated regularly. To operate within the legal requirements of the Mental Health Act and can exercise independent judgement in the assessment of mental health conditions.
To ensure that there are plans in place for those patients that may have ongoing physical health conditions and yet are to be discharged from Mental health services.
Maintain relevant clinical and pharmaceutical knowledge, critically appraise and apply information in practice.
Actively promote health education that may include providing alternative materials to support service users and carers in making decisions regarding their care and treatment.
To provide relevant support to carers and families in relation to their rights as carers and actively support their involvement in the care of the service user.
To adhere to lone working practices.
Communication:
Communicates the patients' health status using appropriate terminology, format, and technology. Use excellent communication skills to provide accurate information to service users and their families and carers in a sensitive and professional manner. Use and demonstrate sensitive communication strategies to ensure that service users are fully informed and consent to care/treatment. Act as an advocate for service users. Maintain working links with other agencies both within mental health and outside to ensure smooth transitions of care for patients. Liaise and communicate effectively with multidisciplinary and primary care teams across health and social care boundaries. To complete all documentation in a timely manner and to a high standard adhering to Trust standards and service requirements. To ensure that all relevant information is shared within the team and accurately documented in order to maintain safe practice across the service. Effective liaison with all parts of the service including daily communication with those responsible for gatekeeping, bed management and accurate data collection.
To participate in service evaluations, benchmarking, research and audit activities, and to contribute to implementation of service modernisation and improvement projects. To participate in clinical governance initiatives as agreed by the directorate clinical governance committee. To support a culture of learning and continuous service improvement amongst the team.
Professional, Legal and Educational:
To work in accordance with the professional Code of Conduct and ensure that standards of practice are upheld. To actively contribute and utilise the professional skills, expertise and knowledge base of the professional qualification (nursing, social work, occupational therapy) held to provide a multi-professional model of care to service users and carers. To provide specialist assessments and interventions in line with the professional training. To share knowledge and expertise with team members through inter-professional training and team working. To supervise students in line with professional requirements. To receive regular clinical supervision. Maintain and develop own professional knowledge and competence by keeping informed of advances and research pertaining to mental health care. Maintain a safe environment for all staff and visitors, taking reasonable care to avoid injury. Co-operate with the Trust to meet statutory requirements. Provide statements regarding untoward incidents, as requested by line manager.
Person Specification
Qualifications
* Relevant professional qualification- Mental Health Nursing, Social worker, Occupational Therapist with affiliated professional registration NMC, HCPC
* Evidence of continuing professional development
Experience
* Experience of working with people in a mental health crisis
* A range of relevant post-registration experience in a mental health setting
* Experience of audit and research
* Experience of alternatives to psychiatric admissions
* Experience of a variety of psychological and social interventions
* Experience of delivering teaching
Skills and Abilities
* Excellent knowledge of mental health policy including relevant legislation especially the Mental Health Act
* Sound knowledge and understanding of current issues in specialty
* Excellent interpersonal, communication, negotiation and influencing skills demonstrated by the ability to engage others
* Excellent written and verbal skills
* Knowledge of the principles of equal opportunities and anti-discriminatory practice in the provision of services
* Sound knowledge of psychopharmacology
Other Requirements
* Empathy for service users including individuals who have experienced mental health problems.
* Ability to fulfil all the requirements of the role (including physical)

Salary: £42,939 to £50,697 a year per annum inclusive of HCAs.
