Essential Requirements:
* Provide essential support to individuals with diverse physical and mental health needs.
* Conduct in-depth assessments and create tailored care plans to enhance client well-being.
* Implement and monitor interventions, ensuring they meet clients’ needs effectively.
* Collaborate with other professionals and agencies to deliver coordinated care.
* Advocate for clients’ rights and needs within the community.
* Demonstrate strong assessment, care planning, and communication skills.
* Work both independently and in a hybrid model, with on-site availability at least once a week.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
