Job summary
As a CommunityNurse working in Runcorn and Widnes you will be integral to the care of thepatients during their time with Bridgewater Community Trust is focused onsupporting the Community and helping patients remain independent in their ownhome. You will actively support the District Nursing team by undertakingresponsibility for a delegated caseload. Undertake the assessment, planning andevaluation of patients care needs. Liaise with the District Nurse team toensure information sharing in relation to patient care / changes to care planand the continuity of care and reviewing of complex care packages. You will liaisewith all health care professionals and other agencies to ensure effectiveservice delivery.

Promoting the healthand wellbeing within the family unit and community as a whole.
Performing comprehensiveassessment of patient nursing needs as the named nurse.
Plan, assess andevaluate programmes of care according to changing health needs under thedirection of the District Nurse.
To support the team leader in the co-ordinationof complex and continuing care packages.
Maintain accurate records in accordance withNMC.
Attend a multi-disciplinary case conference asthe named nurse if appropriate.
To communicate andcollaborate with other members of the PHCT, voluntary and statutoryorganisations to support individual patient care.
To establish andmaintain effective communication with patients, relatives and carers in complexand potentially challenging situations.
To competentlyperform clinical skills related to the post.
To be professionallyresponsible for a delegated caseload under the supervision of the DistrictNurse.
To assess healthneeds at individual, practice and community level.
To undertake chronicdisease management reviews using appropriate template.
To work without direct supervision

Youll need to have the following qualification/experience:
A minimumof 6 months paid experience in a community nursing environment within a CommunityTrust in the last 3 years
Comply withNMC Professional Registration and keep up to date with current nursingpractices
Ensureaccurate records are kept in accordance with CHS policy and NMC guidelines
Undertakeregular mandatory training
*Depending on the Trust, you may require variable additionalcertification and skills.
