All About You
* Qualifications: Registered as a pharmacist with the General Pharmaceutical Council (GPhC).
* Clinical Experience: Ideally experienced in a hospital setting with knowledge of clinical governance and medication management.
* Analytical Skills: Skilled in assessing medication safety, monitoring patient responses, and adapting medication plans accordingly.
* Communication: Effective interpersonal skills to engage with patients, families, and the multi-disciplinary team.
* Problem-Solving: Proactive in identifying issues and developing solutions to ensure safe and efficient pharmaceutical care.
* Organisation: Able to manage and prioritise workload in a fast-paced clinical environment.
* Commitment to Learning: Enthusiastic about ongoing professional development to stay up-to-date on pharmaceutical advancements and clinical guidelines.
What Will You Be Doing?
* Delivering clinical pharmacy services across the hospital, including advising on medication management and optimising drug therapy for patients
* Conducting comprehensive medication reviews to ensure safe, effective, and personalised treatment plans
* Collaborating with healthcare professionals, providing expert advice on pharmaceutical care, and contributing to multidisciplinary team meetings
* Monitoring patients for side effects and ensuring timely adjustments to medications as needed
* Supporting medication reconciliation, reducing errors, and promoting best practices in prescribing and administration
* Educating patients and their families on medications, including potential side effects, to enhance adherence and outcomes
* Contributing to audits, research, and quality improvement initiatives to elevate standards of pharmaceutical care
