Job title: Relief Practitioner | Care at Home Service
Responsible to: Team Leaders and Service Manager
Hours of work: This role offers flexible working including mornings, evenings and weekends
Salary: £12.00 per hour plus annual leave allowance
Location: Field-based working across all three localities of Aberdeen City (base office is Bridge of Don)
Main Responsibilities
Lead and participate in the provision of a high standard of person-led care and support for service users in their own homes, in line with individual care plans and risk assessments, organisational policies, national standards, codes of practice and legislation. Communicate effectively with service users, families and multi-disciplinary teams, record appropriate care notes and create a safe, positive and enabling environment.

Requirements
Job activities
* To understand the physical, emotional, cultural, and social needs of the service users, including a wide range of needs, illnesses and disabilities
* To provide a wide range of domiciliary care including personal and continence care, managing medication, household tasks and meal preparation as agreed in individual care plans
* An understanding of palliative and end-of-life care
* An understanding of working with vulnerable elderly people
* Working to a person-centred and enabling approach
* To observe and promote the service user's choice, independence, dignity, privacy, fulfilment and other rights
* To actively support other Support Practitioners
* To undertake the tasks detailed in the service user's care and support plan using trauma-informed practices
* To encourage the independence and motivation of the service user and foster improved levels of self-sufficiency
* To provide input into the care and support plans alongside the service user
* To use manual handling equipment safely and correctly
* To take responsibility for the safe handling of property and equipment belonging to the service user
* To establish and maintain good communication and develop effective working relationships with service users, families and multi-disciplinary teams
* To ensure as safe as possible the living environment for the service user, whilst respecting the service user's choice and rights
Essential
* A minimum qualification of N/SVQ3 in Adult Health and Social Care, or equivalent professional qualification (nursing etc) (or equivalent professional experience and a willingness to work towards SVQ3)
* Literacy and numeracy at a level to maintain all the appropriate paperwork and report writing.
* Able to use a smartphone app and computer applications such as Word
* Proven track record in a similar role
Systems
* Maintenance of records - Utilisation of PASS to record all information in relation to Service User Case Management
* Team meeting attendance
* Office 365 and IT Platforms
Quality Standards
* Working to the appropriate Care Inspectorate standards
* Working to the appropriate Scottish Social Services Council standards
* Working to the appropriate Office of the Scottish Charity Regulator standards
* Working to the appropriate Aberdeen Cyrenians standards and working in partnership with the commissioner.
* Working to the appropriate Environmental Health standards
