An exciting opportunity has arisen for an experienced HCSW to join our Intermediate Care Team as part of the Integrated Community & Support Services (ICASS).
The Intermediate Care Service provides rehabilitation to the adult population within the locality. This will be delivered to service users as in-patients, out-patients, or within their own home.
The primary function of this role will be to work with the Physiotherapy Service within Intermediate Care to deliver therapeutic treatment programmes to service users in the ward setting or their own homes, preventing hospital admission and supporting early discharge from hospital. Therapists will provide specialist assessments to this patient group and allocate work to you.
You will have an SVQ Level III in Health and Social Care or equivalent. Experience working as a Physiotherapy support worker would be desirable. It would be beneficial to have experience working with older people as they are the main group of service users referred. Working as part of a supportive multidisciplinary team, you will be expected to carry out ongoing treatment programmes for a mixed caseload of mainly older people.
You will have good organisational skills, excellent communication skills, and be able to work independently as well as within the team. There may be occasions where you will be required to work in one of the other community hospital sites.
Public Holiday working is in operation. Our service offers regular supervision, PDP, and supports training opportunities.
The ability to travel throughout the area is essential as community caseload is core to this role.
