An exciting new role has been developed within the innovative and welcoming patient flow team giving the successful candidate a unique opportunity to work with the wider MDT to embellish the experience patients and improve efficiency of patient flow throughout the hospital.
Experience in oncology and patient flow are desirable however not required as we are here to help you develop in the role.
• To support the Acute Oncology Team Leader and patient flow managers in managing the flow of patients through the trust and moving the bed management process forward.
• To work with the multi-disciplinary team to provide real time information to patient flow managers regarding bed status.
• Facilitate flow of patients through the hospital by transporting patients from one care setting to another with direction from registered nurses
• Support care settings in preparing for admissions by preparing bed spaces, communicating with nursing teams.
• Facilitate transfers of patients from one care setting to another by taking/giving handover from one team to another, escorting patients who require nursing escort, carrying out clinical tasks to prepare patients for transfer and liaising with relatives regarding transfers.
• Assist staff in care settings with clinical tasks to allow them to safely accept transfers.
• Orientate patients to a new care setting upon arrival and assist staff with admission process where necessary.
• Co-ordinate the transfer lounge including taking responsibility for care of patients in the lounge.
• Managing admissions to lounge including taking handover and assessing patient suitability for the lounge.
The patient flow team is a friendly and welcoming team who work closely and have good relationships with a wide range of departments across the trust including both inpatient and outpatient clinical settings.
We work in close contact with the hotline team who provide a 24 hour help and advice line to the trust as well as the acute ambulatory care unit and acute assessment unit.

DUTIES AND RESPONSIBILITIES
1. Support, deputise for and taken direction from the patient flow managers
2. Act as a role model to and co-ordinator for patient flow facilitator and AOMS senior HCAs
3. Participate in patient care after appropriate delegation and under the leadership of the patient flow managers or registered nurse in the appropriate clinical area.
4. Alert a registered nurse promptly of any concerns or changes in the patient’s condition
5. Work co-operatively as part of the multi-disciplinary team to deliver a high standard of holistic patient-centred care
6. Implement programmes of care and contribute to care plan modifications, undertaking risk assessments, as directed and within the limits of own knowledge and competence
7. Involve patients in shared decision-making and obtain consent before undertaking and nursing procedures
8. Act in ways that acknowledge and recognise peoples’ expressed beliefs, preferences and choices, respecting diversity and valuing people as individuals
9. Administer medication as outlined in the Trust Policy and as own personal and professional limitations allow
10. Perform agreed area specific procedures and clinical skills appropriate to the clinical area in accordance with the trust policy
11. Ensure thorough and accurate documentation of nursing interventions
12. Communicate effectively with colleagues, providing clear verbal and written information and instructions when sharing information, delegating or handing over responsibility for care
13. Collect/record data accurately in accordance with Trust guidelines, policies and standards
14. Assist in the coordination and safe admission, transfer and discharge of patients in accordance with bed management procedures and Trust policies
15. Participate in the emergency care of patients and assist with/commence basic cardio-pulmonary resuscitation procedures as required
16. Be aware of the physical, psychological, social, cultural and spiritual needs of the dying patient; assess, lead and implement appropriate care management, as required, in conjunction with a registered nurse
17. Assist with the referral of patients to internal/external Allied Health Professionals and/or other agencies
18. Ensure a welcoming, caring and safe environment is provided for the patient and visitors
19. Handles patient’s personal possessions, ensure cash and valuables kept securely as per policy
20. Give support to registered nurses in the provision of a safe and caring environment
21. Organise, prioritise and adjust own workload to enhance the delivery of patient care
22. Contribute towards ward/department organisation
23. Support and assist in the induction and training of Health Care Assistants at Band 2 & 3, acting as a role model
24. Identify potential risks for all staff, patients and visitors and adhere to relevant incident reporting procedures taking appropriate action/advice as needed
OPERATIONAL
1. To liaise appropriately with colleagues regarding bed status and escalate accordingly to senior nurses including providing relevant daily reports
2. Have and awareness of the bed manager’s responsibilities within the major incident plan and its specific role within the Trust wide plan.
3. Liaise closely with all Ward Managers/Nursing Teams and Matrons to ensure accurate up-to-date knowledge of patient placements is achieved.
4. To work closely with the infection control team. Inform them of any high-risk patients admitted to the hospital, and to facilitate the movement of any newly diagnosed high-risk inpatients within the hospital to the correct location
5. Liaise with other hospital Trusts to appropriately repatriate patients from other Trust sites and manage the requests for inter hospital transfers working closely with medical teams to identify clinical priority.
6. To co-ordinate and where necessary take responsibility for care of patients in the transfer lounge
7. Work with and where necessary cross cover the AOMS senior HCAs to support the Hotline Team who provide a 24hours help and advice line to the trust to provide a safe admission process for emergency admissions
TRAINING & EDUCATION
1. Complete appropriate clinical skills and mandatory training as directed by management
2. Participate and undertake research and audit which will influence practice
3. To be up to date and competent in all appropriate areas of clinical practice including medicines management, moving and handling, assessment of critically ill patient and competent in performing ECG’s
4. Attend all mandatory study days
5. Maintain awareness and competency within the role against national and regional initiatives and subsequently implement these initiatives into practice.
PROFESSIONAL
1. Act as a resource giving advice and support to nursing staff regarding emergency admissions and associated issues
2. Liaise closely with the medical team and other members of the MDT in respect of patient care promoting the highest standard of care
3. To develop and maintain a professional interest in all aspects of nursing and associated research
4. To highlight areas of improvement/development within the department and undertake relevant projects
5. Foster and demonstrate creativity, energy and inspiration into ward teams toward efficient patient flows and pathways.
