Therapy Support Worker - Neuro-rehabilitation Service

The post is based in the Neuro-rehab Service. The team works across inpatient, community (Early Supported Discharge ESD) and outpatient settings to provide specialist, evidence-based Neuro-rehab to patients following stroke or other Neurological diagnosis. This post is based on our inpatient unit supporting the team to deliver specialist neurorehabilitation at the beginning of the patients rehab journey. You will also be supported to acquire knowledge and skills to give you a sound core competency in Stroke care and Neuro-rehab.
The inpatient team is an integrated physiotherapy and occupational therapy team working on a 24 bedded ward at North Devon Hospital. They work closely with the broader MDT including the Nursing staff, Doctors, Speech and Language Therapists, Dieticians and Psychologists. The team is specialist in providing acute therapy post-stroke and neurological diagnosis. They assess patients' physical and cognitive abilities as well as functional ability with tasks such as walking, washing and dressing, and activities in the kitchen. They consider the patients' previous abilities, home and lifestyle to plan goals and formulate a treatment plan. The team works closely with patients and families to support them with what has happened and plan for the future. They work collaboratively to plan discharge into a community setting as soon as patients are able to receive their rehab in the home environment.
The community ICSS (Integrated Community Stroke Service) links closely with the Ward team to provide a seamless transition of care and visit patients in their homes as soon as patients are able to be discharged from hospital providing intensive, specialist Physiotherapy and Occupational Therapy.
Working Pattern: 22.5 hours per week. 3 days a week: 08:00 to 16:00. One weekend shift (08:00 to 16:00) in every 6 weeks.
