Rate of Pay: £18.66 - £25.51 per Hour
Summary:
We are seeking dedicated Social Workers to join our Adult Social Care team. The role involves assessing individual needs, coordinating support plans, and promoting a strength-based approach to social work practice. The successful candidate will work closely with individuals, families, and partner agencies to achieve positive outcomes and ensure people have access to the right support services.
Responsibilities:
1. Assess, plan, and review individual needs, ensuring a person-centred approach.
2. Promote independence and well-being, encouraging self-management of risks and needs.
3. Identify reablement opportunities and facilitate access to support services.
4. Support individuals in accessing community and natural support resources.
5. Monitor and review service provisions, ensuring effective multi-agency coordination.
6. Provide professional advice, information, and support to individuals, carers, and colleagues.
Qualifications:
1. A recognised Social Work qualification (Degree/Masters in Social Work or equivalent).
2. Registration with Social Work England.
3. Completion (or willingness to undertake) Assessed and Supported Year in Employment (ASYE) (Band A).
4. Evidence of Continual Professional Development (CPD).
5. Additional qualifications such as British Sign Language or Deaf/Blind certification (desirable).
Experience:
1. Experience in care work, social work practice, or a related field.
2. Ability to manage a caseload and prioritise workload effectively.
3. Strong report writing, record-keeping, and communication skills.
4. Experience of working with diverse individuals and communities.
5. Knowledge of Adult Social Care legislation and policy frameworks.
