What you will do:
1. Manage a diverse caseload, conducting thorough assessments, developing and revising Care & Support Plans.
2. Prepare for social work contact by reviewing case notes and liaising with other professionals for comprehensive information.
3. Undertake assessments of support needs, especially in challenging circumstances, and assist in screening referrals.
4. Implement and evaluate Care & Support Plans to address social, health, and well-being issues.
5. Guide Service Users, Carers, and Families in making informed choices for Self-Directed Care and Individual Budgets.
6. Address Safeguarding concerns proactively, adhering to relevant policies if abuse is suspected.
7. Manage risks effectively and contribute to the development of positive behaviour changes.
8. Respond to crisis situations with the appropriate legal interventions.
9. Maintain accurate records and uphold data quality and confidentiality on social care systems.
10. Prepare for and participate in meetings, decision-making forums, and provide advocacy support when needed.
11. Develop strong partnerships with health, voluntary sector, and other agencies.
12. Stay adaptable and embrace new working practices, especially pertaining to ICT advancements.
13. Engage in formal supervision sessions and contribute to continuous professional development.
14. Identify service improvements and provide constructive suggestions.
15. Promote equality, diversity, and the council’s policies in all work engagements.
