We are a small friendly team whose clinical team includes GPs, Paramedic, Pharmacy Technician, Practice Nurse, Healthcare Assistant, TNA and Social Prescribers. We are also a training practice.
The practice serves over 4,000 registered patients.

Our Nurses play an essential role in providing a caring, friendly and positive service to patients and visitors.
Job Summary
To be responsible for the delivery of nursing services, working as part of the practice multidisciplinary team, delivering care within nurses' scope of practice to the patient population. The general practice nurse will be responsible for several clinical areas such as health promotion, chronic disease management, health prevention, as well as supporting the management team in the reviewing of clinical policy and procedure.
Main Duties of the Job
Key Responsibilities:
1. Develop, implement and embed health promotion and well-being programs.
2. Support the clinical team in the development of individual treatment plans for chronic disease patients.
3. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on the patients' health.
4. Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.
5. Provide wound care (Doppler etc.) to patients.
6. Refer patients directly to other services as appropriate.
7. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
8. Maintain chronic disease registers.
9. Implement vaccination programmes for adults and children.
10. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual).
11. Deliver opportunistic health promotion where appropriate.
12. Maintain professional registration and adhere to revalidation requirements.
In addition to their core responsibilities, the general practice nurse may be requested to:
1. Contribute to the implementation of an effective training program for staff and students within the practice.
2. Support and participate in shared learning within the practice.
3. Continually review clinical practices, responding to national policies and initiatives where appropriate.
