Working as a Recovery Coordinator, you will support clients through one-to-one work, care planning, risk assessments, and facilitating multi-agency collaboration. This role involves engaging with clients in individual sessions and group interventions to ensure their overall health and well-being.
Responsibilities include, but are not limited to:
* Working closely with colleagues, community organisations, and external stakeholders across a range of settings to support clients in maximising their potential.
* Developing and maintaining relationships with external partners to ensure coordinated and effective client support.
* Conducting detailed assessments to identify the needs and risks for each client.
* Developing personalised care plans outlining actionable steps for improvement in the client’s health and well-being.
* Regularly monitoring and reviewing client progress, adjusting plans to meet evolving needs.
* Facilitating group interventions that focus on improving psychosocial outcomes, resilience, and well-being.
* Delivering evidence-based psychosocial programs designed to support long-term recovery and positive client outcomes.
* Providing integrated support, addressing clients' mental, physical, and emotional needs to promote overall well-being.
