As part of the Cancer Data Team, the Patient Flow Coordinator will lead on the tracking of pathways of patients who are suspected of having or have confirmed cancer. By working closely with the (multi-disciplinary team) MDT Clinical Lead, Pathway Manager, and Service Managers and providing a high standard of administrative support, the post holder will help to ensure that cancer patients receive treatment as soon as possible and national cancer targets are met.
The post holder is responsible for providing a high standard of professional and patient-focused service to a range of Trust-wide specialties and will support MDT functions on a day-to-day operational basis.
Please take into consideration that this vacancy requires shift patterns between 8:00 and 18:00 Monday to Friday and is office-based.
Main duties of the job
This position is an opportunity to directly improve the care of cancer patients. By monitoring and reviewing patient care pathways and/or treatment progress, the successful applicant will have the opportunity to work closely with administrative and clinical colleagues to identify and suggest improvements to ensure cancer patients receive treatment as soon as possible.
The successful candidate must be proactive - able to identify ways to improve the progress of patients' treatment pathways and then must take the appropriate action to achieve this, by working closely with clinical teams and managers.
Strong communication skills and the ability to work with others in different roles within the Trust are key to this position, as is strong administrative experience and the drive to improve patient care.

Job responsibilities
Operational
1. Proactively manage a case load of patients to ensure treatment of confirmed or suspected cancer as soon as possible by:
2. Monitoring and reviewing patient care and/or treatment progress using the Patient Tracking List in line with Trust and local policies.
3. Identifying from the Tracking List next steps in the patient care pathway in line with clinical need (peer review).
4. Receiving patient referrals and administering in line with timings and requirements as set out in Trust and National policies.
5. Updating on all enquiries regarding patients' care and/or treatment on a cancer pathway from external Trusts and General Practitioners in a timely manner.
6. Developing an understanding of milestones of allocated pathway for patients within designated tumour group.
7. Participating in national cancer audits and collecting data as required in MDT meetings, allowing the Trust to identify and improve treatment in patient cohorts.
8. Identifying and suggesting improvements that can be made to the patient care pathway and/or the patient pathway tracking process.
Communication
1. Attending MDT meetings and completing actions regarding the care of patients within the designated pathway.
2. Developing working relationships with clinicians and managers across the directorate.
3. Monitoring patients on their pathway and proactively finding resolutions to improve the speed of treatment by working directly with clinicians and managers.
4. Actioning day-to-day issues, ensuring a resolution. Escalating serious issues to the Pathway Manager.
5. Updating patient information on the Cancer PTL system (Somerset Cancer Registry) in line with local policy, to ensure efficient pathway progress.
6. Ensuring other users update the Somerset Cancer Registry in the required timeframe within the designated pathway. Escalate any issues and breaches of the Cancer Waiting Times standards to the relevant management in line with agreed escalation procedures.
7. Reviewing and supporting the development of Standard Operating Procedures, to ensure these are adhered to.
8. Reviewing and supporting the development of internal targets, to ensure these are adhered to.
Staff Management
1. Providing day-to-day supervision and oversight of duties for the MDT Administrative Assistant.
2. Ensuring the Administrative Assistant completes preparation for each MDT.
3. Providing support for day-to-day queries from Administrative Assistants.
4. Ensuring MDT Administrative Assistants are fully adhering to the appropriate Trust policies and protocols.
5. Identifying training and development needs of staff, taking these forward where appropriate.
Other
1. Working within a multi-disciplinary team in developing the service in line with departmental plans and Trust corporate objectives.
2. Undertaking any general office responsibilities including collection and distribution of faxes and post, sorting of incoming post into departmental and priority order, date and receipt stamping of any post.
3. Actively participating in department development as appropriate and participating in departmental meetings.
4. Undertaking any other duties commensurate with the grade, required to ensure the smooth and efficient running of the department.
5. Undertaking any training as required to complete the tasks associated with the job role.
6. Ensuring that all health records are appropriately tracked and securely stored whilst in the department according to the Health Records Policy.

Person Specification
Qualifications
* GCSE or equivalent including Maths and English (C grade or higher)
* A-levels, HND or equivalent relevant experience.
* Previous experience of delivering to high standards in a fast-paced high-volume customer care environment.
* Previous experience working in a hospital or care setting.
Skills
* Excellent interpersonal skills with the ability to liaise effectively with a wide range of multidisciplinary staff.
* Demonstrable experience of data collection, presentation, report writing and handling of data quality issues.
* Knowledge of medical terminology.
* Experience of an administrative role within the NHS.
* Knowledge of cancer waiting times and their application.
