Clinical Healthy Child Practice Manager (HCPM) for the Woodbridge and Kesgrave, and Framlingham and Leiston Health Visiting teams, based at Kesgrave Family Hub (22 Bell Lane, Kesgrave, IP5 1JQ). Main duties of the job As Healthy Child Practice Manager, you will: be at the forefront of managing change, driving performance and continuous improvement whilst leading high-quality services and effective ways of working lead the teams in their planning and reviewing of service delivery to ensure that this is of high quality to achieve the best possible outcomes for children and their families directly manage and lead staff, alongside resources and projects which focus on the social, emotional, physical, and intellectual wellbeing of children and their family, from pregnancy to 5 years through the delivery of the Healthy Child Programme and the Childrens Centre Statutory Framework as outlined in the Health and Childrens Centre Standard Operating Procedures (SOP) and in accordance with our clinical governance framework alongside other public-sector partners, voluntary sector, and communities to engage and enable community led activity through the Health and Childrens Centres, developing resilient communities which rely less on public services. About us The team Suffolks Health (0-19) and Childrens Centres provide services for both prospective andexisting parents/carers, children, and young people, focusing on the earliest point ofintervention to enable the best outcomes for children. Supporting the delivery of theHealthy Child programme, a prevention and early intervention public health programmethat lies at the heart of the universal service for children and families and aims to improvehealth outcomes and ensure those at risk are identified at the earliest opportunity.
