In the community, we deliver services that focus on people with long term conditions living well outside hospital, through our integrated locality teams working with district and community nursing teams. Community members of staff also provide care to patients in their homes and from a number of clinics, health centres and GP surgeries.
Job overview
This is a new post and offers an exciting opportunity to join the innovative POPS team (Perioperative Older People Service). The team sits jointly with the Urology and General Surgery Teams.
Taking part in the on-call rota is optional for the post holder.
Main duties of the job
The POPS team provide preoperative assessment and optimisation for high-risk patients undergoing elective and emergency surgery and follow the patient through the surgical admission coordinating and delivering medical care, rehabilitation and discharge planning. It is a multidisciplinary team comprising consultants, clinical nurse specialists and therapists. The team will be fully embedded at ESHT, with established pathways of care, close collaboration with frailty, anaesthetic and surgical colleagues.

The post holder will undertake Comprehensive Geriatric Assessments, Inpatient Ward Rounds for Frailty patients and take part in Service Development.

Person specification
Qualifications
* Hold a medical qualification or qualification registered with the GMC
* Entry onto the Specialist Registrar for GMC or be within six months of obtaining CCT/CCST or equivalent
* On specialist register for GIM and geriatric medicine
* Appropriate higher medical qualification i.e., NRCP/MRCS or equivalent
* MD or equivalent higher degree of evidence of relevant research
Experience
* Comprehensive clinical experience in general medicine and geriatric medicine
* Experience in Frailty & Comprehensive Geriatric Assessment
Other
* Ability to work in a multi-disciplinary team
* Ability to work in partnership to deliver a patient centred service
