What the Job Involves The role involves acting as a primary contact for residents and their unpaid carers, assisting them in maximizing independence and utilizing community resources. You will arrange community-based care packages, develop personalized support plans, and promote well-being initiatives. Key responsibilities include leading Discharge to Assess processes, conducting comprehensive assessments in line with the Care Act 2014, and performing Mental Capacity Assessments. You will manage documentation and authorizations, adhere to safeguarding responsibilities, and conduct risk assessments. The position requires collaboration with various stakeholders to ensure comprehensive support for service users and compliance with the council’s policies and procedures. Participation in training, development activities, and adherence to health and safety protocols is also expected.
