We are looking for an experienced PracticeNurse to join our teams at the Swingbridge and Welby Practices, based inGrantham and Bottesford, starting this November. This full-time role offers adynamic blend of responsibilities: days per week dedicated to carecoordination and days per week focused on practice nursing.
Main duties of the job
Position Overview:
Care Coordination ( days per week): Take on a pivotal role inmanaging and coordinating care for patients, ensuring smooth communication andcomprehensive support. You will work closely with the Integrated NeighbourhoodTeam and Primary Care Network to proactively engage with patients at risk ofhospital admission or health deterioration. Your responsibilities will includeliaising with Home First Teams, supporting care home residents, and being a keycontact within the GP Practice environment.
Practice Nursing ( days per week): Provide routine nursing care,including routine immunisation schedules, cervical screening, contraception reviewsand family planning, travel vaccinations, chronic disease management, dementiareviews, cancer care reviews, health promotion and disease management.

NMC Registered
Qualified Nurse
Good understanding of QOF, Enhanced Servicesand CQC/NICE guidelines.
Good IT, organisational, communication andclinical skills
Experience of working within anursing team essential
Experience of working in primarycare desirable
Experience of working in a GPpractice desirable
