We are commited to expanding our existing 7 day acute frailty services further with the addition of a focused frailty assessment unut and in collaboration with our community colleagues a hospital at home service across County Durham and Darlington. We therefore have opportunities for colleagues to not only work in, but be involved in the development of the services., The post holder will be responsible for delivering a high standard of care to patients and their loved ones in there own home. They will organise and plan care, depending upon the patients needs ensuring all care needs are met. They will be accountable for their care and supported by colleagues. The care given will be in a patients own homes and will require those involved to travel within a geographical area.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
