About the Company - Merco Medical Staffing is representing a client seeking a Primary Care Pharmacist to join a dynamic, multidisciplinary team. This exciting position will involve working across three sites in Milton Keynes, offering an opportunity to contribute to patient care in diverse settings. About the Role - The post holder will be an integral part of the primary care clinical team, serving as an expert advisor on medicines management, repeat prescriptions, medicines optimisation, and pharmaceutical care. As a key member of the team, the pharmacist will contribute to enhancing patient care by ensuring the safe and effective use of medicines, supporting clinical decision-making, and providing advice on the optimisation of treatment plans.
Responsibilities
* Patient Care: Provide remote and face-to-face services to registered patients, offering advice on medicines, repeat prescriptions, and general pharmaceutical queries.
* Prescribing Support: Assist the primary care teams across the three sites with prescribing, high-risk drug management audits, and service requirements to ensure safe, effective patient care.
* Patient Assessment: Use critical thinking and communication skills to assess patients' knowledge and factors (social, cultural, economic), tailoring advice to individual needs.
* Prescription Management: Prescribe urgent medications using summary care records to ensure accurate re-prescribing.
* Pharmaceutical Advice: Communicate pharmaceutical advice clearly to patients, tailored to their individual needs.
* Clinical Risk Management: Identify and manage clinical risks and health queries in line with professional responsibility, ensuring patient safety.
* Child & Adult Protection: Act on child and adult protection issues arising from health and medicines calls, following established protocols.
* System Documentation: Record accurate, timely information and advice within clinical systems to ensure efficient workflow.
* Evidence-Based Advice: Use electronic databases to provide accurate, evidence-based advice on medicines management.
* Collaboration: Work collaboratively with colleagues across all sites to support service delivery and engage in quality improvement initiatives.
* Regulatory Adherence: Uphold General Pharmaceutical Council (GPhC) standards of ethics, conduct, and performance in all aspects of practice.
* Expert Resource: Serve as an expert resource for clinical and non-clinical staff on pharmaceutical and medicines-related matters.
* Compliance: Follow company policies, protocols, and guidelines in your practice.
* Outcome Evaluation: Evaluate outcomes of practice and interventions, reporting any service shortfalls.
* Incident Reporting: Record incidents or complaints promptly in the system.
* Service Development: Provide advice on pharmaceutical practices and service development within the scope of practice.
* Professional Development: Participate in ongoing professional development and continuous improvement of the service in collaboration with the MDT and Clinical Lead.
Qualifications
* Registered with the General Pharmaceutical Council (GPhC).
* Successful completion or expected completion of the CPPE Pathway.
* Extensive experience working within a primary care setting.
* Proven ability to provide patient care through both face-to-face and remote consultations.
* Demonstrated ability to manage workload effectively while supporting pharmacy-related tasks within a clinical service.
