If so, this opportunity is for you We have an exciting opportunity for a Community Staff Nurse to join our Community Intervention Service on a part-time basis. We are an innovative service covering the Staffordshire locality and we are seeking dedicated, compassionate and highly motivated nurses. As a Community Staff Nurse you will deliver a high quality service, working closely with GP's and multi-disciplinary teams in addressing a range of care issues across the spectrum of health conditions, and preventing avoidable hospital admissions. Within the community we are seeing more complex and acute patients than ever before, with increasingly specialist treatment and care being provided by highly skilled staff. Working within our Community Nursing Team you will support, assess and treat patients with a variety of health needs, delivering high quality care in line with evidence based practice. Due to the nature of community working, ability to travel across the patch to provide care to patients is essential. You must be a qualified nursing practitioner with current NMC registration. Previous experience of working in the community is not an essential requirement. You will be required to work flexibly on a rota system across a 7 day working week. Main duties of the job Provide high-quality nursing care to our patients in our local community to keep them safe at home, prevent hospital admission and support hospital discharge home. Assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way, whilst maintaining the patient's privacy and dignity.
