The Complex Needs Team are a multi-disciplinary team, made up of Mental Health Nurses, Social Workers, Support Workers & Psychologists. The team is co-located within Harbour Drug & Alcohol Service and work with people experiencing co-occurring mental health & substance use issues. The work undertaken is in a range of environments, including hostels and other voluntary sector services, including those in the Plymouth Alliance.
The post holder will carry a small caseload of clients with the view to supporting access to housing, detox, rehabilitation, as well as undertaking work around harm reduction, emotion regulation, managing distress & life threatening behaviours.
The team receives supervision through reflective practice, CPD and access to regular training opportunities that is offered throughout the Plymouth Alliance.
Main duties of the job
1. The post holder will carry a caseload of clients with co-occurring mental health and substance use issues.
2. To provide line management & caseload supervision to Band 5 & below.
3. To contribute to professional development, appraisal, performance review and recruitment.
4. The post holder will act as a Care-Coordinator for a defined group of patients and organise care in accordance with the Personalised Care Framework.
5. The post holder will ensure that their clinical practice is based on evidence and good practice and in accordance with any NICE guidance.
6. The post holder will be individually accountable for their practice in accordance with the NMC, HPC, Social Work England professional code of conduct.
7. To promote positive risk to allow clients independence and autonomy of choice.
8. To liaise with other community resources and facilitate use of mainstream resources.
9. To maintain a caseload of clients with a significant degree of mental health complexity.
10. To participate in the shift lead and provide appropriate supervision.
11. Practice within the legal and ethical framework set out by mental health related legislation to ensure that service users interests and well-being are at the fore.
12. Liaise, advise, develop and maintain good relationships with service users, carers, relatives and fellow health, social care professionals who are involved in the delivery of services.
13. Engage in support and care plans for those who have complex needs, high risk behaviours alongside dynamic risk assessments and effective management of risk.
14. Work collaboratively with Adult Social Care to identify and implement any eligible Care Act needs.
15. Participate in all activities aimed at keeping professional skills updated and contribute to dissemination of that knowledge and experience to all staff and students within your area of responsibility.
16. Act upon referrals from secondary mental health services, other primary care workers, consultants and other statutory bodies by providing triage, assessment, implementation and development of programs of care to individuals, groups and families, who are difficult to engage in mental health services.
17. Plan, with teaching establishments, a realistic learning experience for students and for postgraduate students and be responsible for the quality of the programme.
18. Participate in and support training programmes designed for all staff groups.
19. Monitor standards of practice within the team, ensuring areas of deficit are addressed through audit.
20. Co-ordinate the collation of evidence within the team in support of standards for better health improving quality.
21. Prioritise and re-assign team workload and develop caseload management system.
22. Assist the Team Manager in the Investigation of complaints made regarding the service.
23. Actively support the team through change in organisation and service delivery.
24. The post holder will attend and lead community drop-in clinics in the homeless provisions across the city.

* Post holder must have a relevant Undergraduate or Postgraduate degree in Nursing, Social Work, Occupational Therapy and a professional registration with one of the following:
* - NMC
* - HCPC
* - Social Work England
* Further professional development in:
* - Recovery CBT
* - Solution Focus Therapy
* - Dialectical Behavioural Therapy
* - Motivational Interviewing

Experience
* Relevant experience of working with adults who use substances in problematic & harmful ways.
* Minimum 12-months experience in a Community Mental Health setting or clear evidence of how candidate is able to fulfil the functions of a Community Mental Health Practitioner.
* Care co-ordination experience.
* Case load management, and managing a complexity of mental health and substance use risk.
* Experience at a senior level which includes managerial experience, staff supervision and operational issues.
