An exciting opportunity has arisen for a Band 4 Health and Social Care Assessor within the Bee at Home team. This role is primarily based at The Orchard Centre, where you will join a team of Health & Social Care Assessors to triage referrals in to the service, carry out patient assessments in their own home and coordinate the shifts, acting as a point of contact to the Health Care Assistants.
Bee at Home are a 7 day week service, operating between the hours of 8am - 10pm, covering Hull and the East Riding. You will be required to cover 3 long fays and 1 short shift.
Assessment experience is advantageous.
