An exciting opportunity has arisen to join the multi-disciplinary Parkinson's disease team at County Durham and Darlington NHS Foundation Trust.
You will act as an advanced practitioner and a specialist resource in both Parkinson's disease and related Neurological Movement Disorders. This will encompass the nursing care and management of people with these conditions, working closely with patients, carers, Consultants and General Practitioners, therapists and social services. The working environment will be in primary and secondary care settings across the trust.
This role is a trustwide position for 37.5 hours a week, covering shifts between 8.30am and 5pm Monday to Friday.
Main duties of the job
The post holder will work as a key member of the medical and multidisciplinary team, providing highly sensitive information, advice, care management and support regarding patients' care and treatment within the hospital and community.
They will both promote and initiate research-based nursing and medical practice.
The post holder will identify and address individual needs, setting and implementing programmes of care and identify and address shortfalls in service provision Trust wide.
The post holder will initiate new specialist support developments within the hospital and community settings Trust wide and take an active part in research and development and use evidence-based practice.
The post holder will be aware of the need to network across the specialty of Parkinson's disease and related neurological movement disorders, both locally and nationally.
They will work in partnership to deliver high-quality care for those with Parkinson's disease and develop links locally and nationally to improve services.
Job responsibilities
To act in an advisory role and take direct responsibility for the assessment of physical, psychological, financial and social support needs of people and carers of people with Parkinson's disease and related movement disorders.
Use specialist knowledge and skills to identify symptoms and problems.
Communicate in a professional and sensitive manner with individuals, their relatives and carers in highly emotive or distressing circumstances taking their beliefs and culture into account. This can be from diagnosis to palliative care.
To take direct responsibilities for providing highly sensitive, accurate, research-based information on Parkinson's disease and related neurological movement disorders including diagnostic treatments, prognosis and disease management. This may be to patients, carers, GPs, medical staff and other health care members.
To take direct responsibility for assessing the need for Neurological/medical review and to arrange review with managing Consultant or GP.
To take direct responsibility for prescribing, adjusting or titrating medication, following assessment, using an agreed treatment plan.
To take direct referrals to nurse-led service from Consultants, Neurologists and other health care professionals and therapists, to be reviewed in nurse-led clinics.
To be responsible for managing own caseload, including in-patients, outpatients and home visits.
Provide nurse-led clinics Trust wide and maintain telephone helpline.
Provide telephone and written advice for GPs on all aspects of disease management and medication changes.
To take direct responsibility for informing patients and carers of specific aspects of care, including physical complications, drug interactions, drug efficacy and side effects.
To be responsible for reviewing patients' response to treatment by regular contact, monitoring and detection of side effects, titration and reduction of medication following agreed treatment plans.
Maintain accurate and updated information to support clinical practice, including maintenance of database.
Act as a resource for Parkinson's disease.
Provide highly sensitive information, support and advice for patients with Sub-thalamic Deep Brain Stimulation.
Counsel patients, carers, family and where appropriate patients' work colleagues giving support and maximum independence.
Provide written individual statements to support claims for benefits or adaptations.
Monitor and assess an individual's ability to drive and, where appropriate inform the relevant party if a patient's condition changes.
Plan, implement and deliver specialist educational support and training for nursing, medical and care staff, in primary and secondary settings Trust wide.
Support both patients and carers at home and remain as key worker if appropriate when they require admission to hospital, nursing or residential care.
Work closely with relevant charitable organisations, providing support for local groups, facilitating integration with local services, NHS and local authority.
Plan, implement and evaluate service provision.
Identify patients suitable for Apomorphine, perform and be accountable for Apomorphine challenge investigation without medical supervision.
Collect, evaluate, analyse, interpret and report on complex information.
Maintain accurate patients' notes, databases and specialist reports in accordance with Trust standards.
Work as an autonomous practitioner, managing case load, exercising high levels of judgement, discretion, problem solving and decision making.
Maintain good awareness of NICE guidelines regarding Parkinson's and work towards the key recommendations, promoting good practice.
