Job summary We are looking for a dedicated Health and Wellbeing Coach to join our team, combining motivational interviewing techniques with psychological wellbeing practices to support patients within Primary Care. In this role, you will help patients take proactive steps to improve their mental health and wellbeing through a supportive, evidence-based approach. You will work with individuals both face-to-face and on occasions, virtually, managing a caseload and delivering mental health care services across the St Helens Central Primary Care Network. Main duties of the job Key Responsibilities: Provide motivational and psychological wellbeing coaching within a Primary Care setting, supporting patients both in-person and remotely. Manage and prioritise a caseload using strong communication and negotiation skills while considering the whole person. Use IT and MDT channels to screen patients and identify those who would benefit most from health coaching.
