An opportunity has arisen for a suitably qualified and enthusiastic individual to join the Stroke Early Supported Discharge Team as a Band 4 Therapy Clinical Support Worker.
This role is fixed term until June 2025 .
This role will suit someone who is passionate about providing high quality care and rehabilitation for stroke patients, working together with the MDT and in partnership with patients and families.
The post holder will support patients to carry out rehabilitation programmes set according to patients aspirations as well as providing information and support to patients and carers about life after stroke and services available for support in the community.
The post will involve an element of screening and assessment and is fully supported with training and a supportive multidisciplinary team consisting of Physiotherapists, Occupational therapists, Speech and Language Therapists and other Rehabilitation Support Workers.
Main duties of the job
To be an integral member of the team Stroke Early Supported Discharge Team team, delivering therapeutic treatments to patients in liaison with senior therapy staff.
To take responsibility for designated elements of the patient treatment.
The successful post-holder must be an excellent communicator and motivator, experience of working within a Therapy setting is desirable.
Proven organisational skills is crucial. An ability to work in close cooperation, liaison and with the broader healthcare workforce is essential. They will have proven ability as a team player.
An ability to take responsibility, be flexible and have a sensitive and empathetic approach is needed. Participation in manual handling tasks is crucial and the post-holder must be able to undertake the physical and manual handling requirements of the post.
