Barnardo's work in partnership with Lincolnshire County Council to deliver its Leaving Care Service for young people aged 16-25 years old. We support, encourage, and empower care experienced young people to live independent lives. Due to the service expanding this has created two new Leaving Care Worker positions, one position based in Lincoln, supporting care experienced young people aged 16 - 21 years old and a further position based in Sleaford to support 21-25-year-old care experience young people, both roles are exciting positions and new additional posts within the service. The 21- 25-year position will include developing a step forward service for post 21 care experienced young people. These positions are under the current Barnardo's contract which will be ending on March 31 st, 2025. Please note, that the service is currently subject to TUPE with the service being insourced back in house to Lincolnshire Country Council. The job role is a permanent position. Initial Specific Responsibilities Within Lincolnshire the Signs of Safety model is used across County as an assessment tool. Offering information, advice, guidance and support with Accommodation, Health, EET, Finances and Independent skills. Carry out Pathway Plans Reviews which include recognising assessing and managing risks in the form of danger statements and safety planning, carrying out statutory face to face visits travelling nationally to deliver these and writing comprehensive supervision overviews of case management. Actively work with post 21 years old care experienced young people to complete a time-framed focused support with Care experienced young people. Supplementary Information Knowledge of the risks worries and safeguarding concerns when working with Care Experience Young People and Unaccompanied Asylum-Seeking young people is desirable. Knowledge or experience of working with Children in Care and Care experienced young people is desirable. Present oneself in professional manner with continued professional development and a desire to learn is essential. Knowledge & or experience of working with young people in a Trauma Informed way. Able to accurately and clearly record case work deploying the necessary standard of writing and IT skills is essential. To have experience of pathway planning with young people and using a Signs of Safety model desirable. Work collaboratively in partnership with other agencies to provide information, advice, and guidance on: Appropriate accommodation, Helping the care experienced young person to acquire independent living skills: Employment, Education and Training, Finance - New homes Grant, Applying and manage Universal credit claims, Asylum claims & Health issues is essential. To work on your own initiative and to assist specialist workers in supporting Care Experienced Young Person to attain their employment, housing, educational and emotional wellbeing aspirations. Good level of knowledge of IT use, Microsoft programmes and internal platforms and systems. Experience of professional report writing skills is essential. Able to self-manage own diary, workload and prioritise tasks and work under pressure is essential. Participation to work as part of a team and participate in delivery of the Urgent Support service is essential. Essential Driving Licence and ability to travel nationwide, occasional overnight stays is essential. Be prepared to work outside of core hours as required is essential.
