Brownlow Health is a large at-scale General Practice, based over four sites in Central Liverpool. We serve 80,000 patients, and are part of Central Liverpool Primary Care Network, which incorporates 9 practices, covering 110,000 patients city-wide. We provide traditional general practice to a diverse inner-city population across a wide demographic, and serve several Inclusion Health groups, including homeless and Refugee and Asylum populations, as well as supporting patients with issues related to substance misuse. We have a large multidisciplinary workforce, including a pharmacy team consisting of a pharmacy manager, pharmacists, pharmacy techs, and prescribing clerks. We are looking for a senior pharmacist to join our pharmacy team to work alongside two other senior pharmacists to lead a large team of pharmacists, technicians, and prescribing clerks.

Job responsibilities
Patient-facing Clinical Medication Review:
1. Undertake face-to-face clinics, domiciliary visits, telephone, and virtual medication reviews.
2. Perform structured medication reviews for patients on both single medications and polypharmacy patients.
3. Review the indication for each medicine, compliance, monitoring arrangements, and ensure patients get optimal benefit from their medication.
4. Make appropriate recommendations as needed.
5. Review and complete shared care agreements.
6. Partake in an on-call pharmacy rota in hours, addressing on-the-day phone calls, queries, and providing medicines support to the duty clinical team.
7. Undertake changes to patients' medication from hospital discharge and clinic letters.
8. Undertake clinical drug reconciliation for new patients registered with the practice.
9. Provide a comprehensive prescribing service to care homes in conjunction with care home MDT and named staff for each care home.
10. Provide services linked to prescribing that improve patient access.
Medicine information to practice staff and patients:
1. Handle relevant medicine-related enquiries from GPs, other practice staff, other healthcare teams (e.g., community pharmacy), and patients with queries about medicines.
Repeat prescribing:
1. Assist GP practices with the implementation and delivery of their repeat prescribing policy.
2. Contribute pharmaceutical advice for the development and implementation of new services linked to medicines management.

Although we allow for some remote working (depending on the role), you will need to be based on-site in one of the Practices to carry out your duties. You will also need to reside in Liverpool (or within the near vicinity).

Experience
* Minimum of 4 years post-qualification experience.
* 2 years experience in primary care.
