District Nursing Sister, Band 6 Bexley Twilight Permanent; Part Time - per week
We are seeking to appoint an innovative Specialist Practitioner District Nursing Sister. You must hold the District Nurse Specialist Practitioner Qualification. You will manage a District Nursing team within the Bexley (Twilight Team) directorate of Oxleas NHS Foundation Trust.
The successful candidates will be part of a progressive and supportive team located in South East London. Our District Nursing Service delivers care to a diverse population across the boroughs of Bexley.
We are looking for an enthusiastic, conscientious, adaptable DN sister and team player with highly developed nursing skills.
You must be able to work on your own initiative. Previous clinical team management experience is essential.
In our district nursing teams, due to the requirement to be able to travel between patients' homes in a timely manner, a full UK driver's licence, with a Business Insurance is essential as well as access to a car for work purposes.
You will be committed to the development of the team through effective leadership and be given the opportunity to develop your own clinical skills through the delivery of a wide range of evidence based care and an active personal development plan.
These posts are not suitable for RMN or Learning Disability nurses.
Main duties of the job
The post holder will lead a team of nurses and support staff with the responsibility of managing the care for a defined caseload, during the out of hours period from 5pm to 11pm..
Community Nurses work as part of a team delivering high quality evidence based nursing care to clients within their own home, residential homes and clinical environments. As part of a District Nursing team you will have a role placing emphasis on prevention and rehabilitation as well as curative and palliative care.
The post holder is an autonomous practitioner and will assess, plan, implement and evaluate patient care and also participate in the training of patients, relatives, carers, students and other health care professionals

5. To assess, plan, implement and provide regular evaluation of health care needs in conjunction with patients, their carers and other appropriate members of the multidisciplinary team.
6. Prioritisation, organisation and appropriate delegation of the responsibilities and workloads within the team will be required to ensure efficient and effective service provisions.
7. To be responsible for the caseload management of patients within the practice population.
8. To be responsible for keeping up to date with current issues and events in Primary Health Care.
9. To actively participate and influence the developments of Community nursing services.
10. To provide support and health education programmes for patients and carers through effective interpersonal skills.
11. To work flexibly and relieve colleagues/ rotate within the trust as required and requested by the line manager to meet the needs of the service.
12. To work alongside other members of the multidisciplinary team attend meetings of Primary Health Care Team.
13. To ensure own mandatory training and team members mandatory training is kept up to date in line with Trust policies.
14. Ability to use Information Technology including Word, email and the internet and RIO.
15. Participation in complaint investigations, when requested.
