The Community Learning Disability Team works with people who have learning disabilities and health conditions that needspecialist care. The key aim is to reduce health inequalities by helping people overcome barriers to good health. The multi-disciplinary team provide specialist health advice to people with learning disabilities, their carers, families and the wider health and social care community. The team can provide community nursing care, occupational therapy, physiotherapy, vision and hearing support and speech and language therapy. Main duties of the job To be responsible for the effective leadership, management, delivery and development of the community learning disability team (CLDT), including LD nursing, physiotherapy, occupational therapy, speech and language therapy, assistant practitioners, and healthcare assistants, in a designated locality. To work with the Head of Service to ensure service development and delivery is in line with KCHFT's strategic and business objectives; and with national and local policy and regulatory standards, e.g. CQC.
