This is a fantastic opportunity for an experienced practitioner who is passionate and committed to providing support to our Care Experienced Young People. The post holders will be responsible for: - Working with young people leaving care to ensure a smooth transition into independence Assisting and supporting young people to prepare for independence and to support them once independent Maintaining accurate records as required on the young person’s file. You will need to have: - NVQ level 3 or above in a Social Care, Education, Health, Youth & Community Service or Early Years. Experience of working with young people experiencing difficulties within their families. Experience of working within multi-disciplined partnerships i.e. housing, welfare rights & careers guidance.
