The NHS long term plan is committed to the expansion of pulmonary rehabilitation (PR) services to respiratory patients. As a result, NHSE released funding to improve referrals and completion, reduce health inequity, and quality assure services through the National Pulmonary Rehabilitation Service Accreditation Scheme (PRSAS). The nationally accredited NWL Inner Boroughs PR Service (under Imperial) has successfully secured funding for a Band 5 Exercise Practitioner to join the service on a 12-month contract.
This new role will help to support the PR lead in addressing service inequity and long-term self-management within the PR service. The successful applicant will support the PR lead and Band 6 Social Prescriber in providing service promotion and awareness in hard-to-reach areas and work with community teams to improve patient care and support uptake. As part of the role, they will provide alternative options for patients who are unable to attend/partake in traditional PR models. This will increase patient choice and enable the service to provide a more holistic, accessible, and equitable service across the region. This will support the service in addressing health inequity and meeting the NHS long term plan. They will also be responsible for supporting the exercise component of the post-PR follow-up clinic with the social prescriber and Band 7 to help improve patient self-management and activity level, thus reducing the risk of deterioration/healthcare.
It is an exciting pilot that will allow the successful applicant to develop skills both clinically and non-clinically in data collection, analysis, and reporting as well as project management and presentation.
Main duties of the job:
* To facilitate and manage the provision of high-quality care to all adult respiratory patients alongside the consultants, AHPs, and nurses in respiratory medicine in a variety of settings to promote respiratory health within ICHNT and the surrounding community.
* To work as an autonomous practitioner in prescribing and monitoring patients' exercise programmes and to maintain up-to-date exercise records.
* To work as an autonomous practitioner in educating patients both prior to discharge and as part of their outpatient rehabilitation programme.
* To use evidence-based principles/current best practice to assess, plan, implement, and evaluate interventions.
