An exciting opportunity has arisen to join our Hospital Flow Directorate Team within the Discharge Lounge Service. These are new roles created to support the timely and safe discharge of our patients from acute services and will be key to ensuring the safe booking and usage of the various transport requirements for patients. The roles will work across the 5 day period. An understanding of the discharge process within a hospital setting would be beneficial, but the key skills required are a professional and approachable manner- both in person and via telephone, and ability to use/ willingness to learn various computer systems. These will include Microsoft office and internal systems. The future post holders will be a key part of the Discharge Lounge team and will be able to show their organisational skills and problem-solving abilities on a daily basis. To ensure that all referrals (discharge notifications) are checked, recorded and inputted on appropriate databases/systems and ensure they are send to appropriate areas. To ensure that recording, processing and updating local authority systems is in line within their specific policies and procedures. Updating sickness and absence procedures. Stationary and equipment orders Information and data processing and reporting Maintaining patient/service user files Updating care plans/assessments Invoices for orders Minute taking Updating Master Control Plan (patients who are medically optimised).
