Job overview
THIS POST IS FIXED TERM FOR 24 MONTHS DUE TO SERVICE NEEDS.
An exciting new opportunity has arisen within the Cardiology and Cardiothoracic directorate for a dynamic and motivated Clinical Nurse Specialist, to work in collaboration with Cardiothoracic Outpatient team, for the implementation, development, and delivery of a new Lipid service clinic, in conjunction with the existing Post Percutaneous Coronary Intervention (PCI) clinic.
Extensive cardiology knowledge and a keen interest in lipid management is essential.
The successful candidate should be organised, efficient, and have excellent communication skills, to work collaboratively with the multidisciplinary team.
We are able to offer a band 6 training position with the aim of successfully achieving competence to move to a band 7, once clinical competencies and objectives are achieved.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
This is a new service incorporating the following duties and responsibilities:
* Initiate, facilitate and participate in the delivery of a new Lipid service clinic, in conjunction with the existing Post PCI clinics.
* Formulate/develop/implement care management pathways, protocols and guidelines.
* Provide teaching, support and supervision to other colleagues.
* MECC (Making Every Contact Count) and encouraging healthy behaviour choices, to maximise clinical outcomes for patients.
* Auditing, and improving service delivery.
This project aims to address gaps in current lipid management practices post-acute coronary syndrome (ACS) at Cardiff and Vale University Health Board. It encompasses the implementation of designated 3–6-month follow-up appointment system, the establishment of a dedicated post Myocardial infarction (MI) /lipid service, patient education and support, and promotion of health-related behaviours.
