Social Worker - Short Breaks Team

Salary: £35,496 - £41,586
We are seeking a dedicated and experienced Social Worker to join our Short Breaks Team. This role offers a rewarding opportunity to make a positive impact on the lives of children and young people with disabilities and their families.
Key Responsibilities:
1. Safeguarding and Welfare: Act as a key member of the social work team, safeguarding and promoting the welfare of children and their families.
2. Statutory Compliance: Discharge the Authority's responsibilities under the Children's Act 1989 & 2004 and other relevant legislation.
3. Professional Practice: Maintain high professional standards, adhering to Social Work England guidelines and the Division's values.
4. Assessment and Service Delivery: Conduct thorough assessments, develop and implement effective care plans, and liaise with other agencies to ensure seamless service delivery.
5. Child Protection and Looked After Children: Work within the London Child Protection Procedures to safeguard children at risk and support those in the care of the Local Authority.
6. Family Support and Prevention: Collaborate with health, education, and targeted family support colleagues to address unmet needs and prevent statutory intervention.
7. Support and Advocacy: Advocate for disabled children and young people, supporting them to access community resources and achieve their full potential.
8. Transition Planning: Facilitate smooth transitions to Adult Social Care services, ensuring continuity of care.
9. Professional Development: Engage in ongoing professional development to enhance skills and knowledge.
Essential Requirements:
1. Social Work qualification (Degree or equivalent)
2. Registration with Social Work England
3. Significant experience in working with children and families, particularly those with disabilities
4. Strong assessment, planning, and intervention skills
5. Excellent communication and interpersonal skills
6. Ability to work independently and as part of a team
7. Commitment to safeguarding and promoting the welfare of children
