Job summary
2 WTE substantive posts.
The role will be based either at The Council House in Derby or Scarsdale in Chesterfield. There is a requirement to travel to GP practices across Derbyshire. Derby & Derbyshire ICB organisation is now working to a hybrid model, with staff splitting their time between practices and working. The Derby and Derbyshire ICB Pharmacy Team are seeking qualified pharmacy technicians with experience of working in primary care to contribute to the development and implementation of medicines optimisation. Candidates should have a BTEC in Pharmaceutical Sciences or NVQ level 3, be registered or eligible to register with the GPhC, and have demonstrable post-qualification experience.

Main duties of the job
1. Working with a number of GP practices across Derbyshire to deliver prescribing related cost-efficiencies and minimise medicines waste in line with agreed standard operating procedures.
2. Providing education, advice and support to practices to improve the safety, quality and cost-effectiveness of medicines.
3. Retrieve, analyse, interpret, and present patient related prescribing and clinical data from practice clinical computer systems and written notes to allow clinical decisions about medicines.
4. Implement prescribing changes and inform the patient, practice staff and local community.
5. Deliver education to healthcare professionals, care home and social care staff, patients and the public to promote effective medicines.
6. Liaise with GPs, secondary care, community social care, community pharmacy, other providers, patients and carers to deliver medicines.
7. Maintain confidentiality at all times in line with data protection and Caldicott guidance.
