Tradewind Australia partners with service providers across the Aged Care sector to offer quality care to Victorian communities.
We are seeking experienced Registered Nurses with a background in Care Advisory roles within the Home Care Packages (HCP) program who are open to either Locum or Permanent roles in various locations across Melbourne.
Key Responsibilities:
1. Comprehensive Assessments & Care Planning: Conduct clinical assessments and develop tailored care plans based on the clients' medical, personal needs, goals and preferences.
2. Clinical Oversight & Service Coordination: Ensure the safe, high-quality delivery of care by overseeing clinical services and coordinating with a multidisciplinary team.
3. Monitoring & Adjusting Care: Regularly review and adjust care plans to accommodate any changes in the client's health status or care needs, ensuring optimal outcomes.
4. Collaboration with Healthcare Providers: Work alongside GPs, allied health professionals and other service providers to deliver coordinated, person-centred care addressing both clinical and non-clinical needs.
5. Compliance & Documentation: Maintain accurate, up-to-date clinical documentation and ensure compliance with Home Care Package guidelines and regulatory standards.
6. Education & Support: Provide clinical guidance and support to care staff, clients and families to ensure safe and effective care delivery.
Key Requirements:
1. Bachelor of Nursing and current AHPRA registration
2. Proven experience in a similar role within the Aged Care sector
3. Extensive knowledge of Home Care Packages (HCP), aged care support systems, and funding mechanisms
4. Strong understanding of the challenges faced by older people, their families, and carers
5. Experience delivering person-centred care and supporting models that promote client empowerment
6. Excellent interpersonal skills with the ability to work in a multidisciplinary team
