Job summary
An exciting opportunity has arisen for a full-time Band 3 Community Support, Time and Recovery (STR) Worker to work within the Autism Spectrum Condition Service - Mersey Care Foundation Trust.
This post will be based at Willis House.
The post advertised will provide support to our intensive support team, ensuring access to health and social support opportunities for people who have a diagnosis of Autism Spectrum Disorder (non-LD).
The successful candidate will work within a MDT setting with support from qualified practitioners from the team and will be expected to work unsupervised at times with people in the community.

Main duties of the job
The post holder will work to assist with the continuity of care for service users within the appropriate care setting. The post holder will work within the community environment.
The post holder will provide support, give time to an allocated group of service users and aim to promote recovery predominantly within the community environment.

Job responsibilities
Under the direction of the Qualified Practitioner, participate in the delivery of planned care for service users in the community.
The post holder will undertake a range of duties which include.
Facilitate and support the development of appropriate personal care.
Facilitate and support self help, communication and social skills acquisition.
Assist and promote independence with activities of daily living.
Facilitate a service users recovery by assisting them to participate in social, vocational, recreational and leisure activities within the appropriate setting.
Promote social inclusion through developing opportunities in areas such as employment, volunteering and education.
Participate in the implementation of specific programmes of care or therapy to develop and enhance socially appropriate adaptive behaviours under the supervision of qualified staff
Participate in regular supervision, engage in reflective practice and be committed to continued development
Provide information to enable the effectiveness of care to be evaluated.
Ensure appropriate records are maintained under the direction of the qualified practitioner.
Contribute to the involvement of Service Users, and carers/relatives in the delivery of care
Provide reassurance and support to service users and carers/family
Work collaboratively within the Care team to ensure that Service Users needs are met
Recognise and respond appropriately to urgent and emergency situations
Contribute to the development of services
Contribute to the effective and efficient use of resources
Help to identify early signs of relapse as described within the care plan and alert the appropriate staff involved in the service users care.
Understand and adhere to Trust policies, procedures and clinical guidelines
Report any concern regarding Service User care to the Qualified Practitioner
Contribute to service improvement initiatives to enhance quality of Service User care
Participate in Service User and public involvement activities
Support individuals equality, diversity and rights
Participate in Serious Untoward Incident investigations/reviews as required

Qualifications

1. NVQ 3 or equivalent level of experience within a care setting
