In this key role, you will be responsible for providing high-quality care and support to individuals, as well as overseeing and mentoring junior support workers. You will play a crucial part in ensuring that the people we support receive exceptional care in a safe and respectful environment. Key Responsibilities: Provide Person-Centred Care: Deliver direct care and support to individuals, focusing on their personal needs, preferences, and independence. Leadership & Mentoring: Support, supervise, and mentor junior support workers, ensuring they follow best practices and meet the standards of care. Care Planning: Assist in developing, reviewing, and updating individual care plans, ensuring they are tailored to meet the specific needs of each service user. Record Keeping: Maintain accurate and up-to-date records for all service users, including daily logs, risk assessments, and care plans. Medication Management: Administer and monitor medications, ensuring compliance with safety protocols and policies. Communication: Liaise with families, healthcare professionals, and other external agencies to ensure coordinated and holistic care. Team Collaboration: Work closely with the Care Manager and the wider team to maintain a high standard of care and to improve care delivery. Health & Safety: Ensure a safe environment for both service users and staff by following health and safety guidelines and reporting any concerns. Key Skills & Requirements: Qualifications: NVQ Level 1 or 2 in Health and Social Care (or equivalent) is essential. Additional relevant qualifications in social care or leadership are desirable. Experience: A minimum of 2-3 years of experience in a support worker role, ideally in a senior or supervisory capacity. Experience working with individuals with specific conditions, such as learning disabilities, autism, mental health. Skills: Strong leadership and team-building abilities. Excellent communication skills and the ability to work collaboratively with a multidisciplinary team. A compassionate and empathetic approach to supporting individuals.
