We are looking for a Band 5 Community Staff Nurse to join one of our well-established Community Nursing teams based at Hawkhurst Community Hospital, to support the development of this service.
Being a well-established team, it can offer experience and knowledge to support own growth. The service operates seven days a week from 08:00 - 16:00, with occasional late shifts of 10:00 - 18:00 on a rotational basis.
The team supports patients from GP practices within the Weald PCN Primary Care Network (PCN) covering the Hawkhurst, Cranbrook, Staplehurst, Marden, Lamberhurst, and Robertsbridge areas.
The teams provide nursing care to predominantly housebound patients where each team takes responsibility for a defined geographical patch, allowing good working relationships with local GPs, care homes, Social Services, voluntary organisations, and other stakeholders.
Main Duties of the Job
The Community Nurses provide local clinics for patients requiring their clinical skills. As the Community Staff Nurse, you are able to support a patient with nursing care delivery towards the aim of self-management.
This is a highly rewarding role, and nurses are supported to achieve all the clinical skills required with local practice educators and extended training.
Clinical areas are diverse, including clinical skills for wound management care, end of life care, medicines administration and IVs, bladder and bowel care, amongst other duties.
Each team has two senior staff, therefore, these posts are suitable for those who are newly qualified or moving from an acute setting.
Staff will be required to drive their own car and have a full, clean driving licence. They are able to claim mileage costs and access the NHS car lease provision, following their probationary period.

Person Specification
Qualifications
* Registered Healthcare Professional with relevant Professional body (1st Level Nurse/ NMC)
* Degree in a health relevant subject or willingness to complete a degree (within 3 years of appointment)
Experience
* Experience of recent community/acute healthcare gained through education or work experience placement or previous employment
Knowledge
* Awareness of professional issues and recent developments in the NHS
* Knowledge of up to date clinical practice
Skills and Abilities
* Sound communication and interpersonal skills (verbal and written)
Personal Attributes
* Ability to motivate and organise others to ensure best practice.
* Valid driving licence and access to a car for work purposes.
* Ability to work under pressure
Ability to Manage
* Able to support the implementation of local and national agenda for health under the guidance of the team lead / ward manager.
* Ability to prioritise care delivery in a professional manner, taking into account Trust policies and procedures.
