The post-holder will be responsible for the implementation, dissemination and support of the Personalised Care Plan for the Last Days of Life within the Trust through direct clinical practice and education/audit. The post-holder will act as a Keyworker for designated palliative / EOLC patients, providing a link between patients and health care professionals across primary, secondary and tertiary care and act as an advocate for patients/families. The post-holder will work with the Palliative Care Team to improve end of life care for cancer and non cancer patients and the EOLC training programme. The post-holder will work with the Palliative Care Team regarding the EOLC tools to ensure they are embedded within practice. The post-holder will work closely with the Acute Oncology Service/EAU Departments, Herts/Beds/Bucks Discharge and In-Reach teams to support the rapid discharge of eolc patients to their PPC. The post-holder will ensure their role reflects the core components of the specialist nurse which includes: clinical practice, management, education, research and audit, taking into account the national EOLC agenda, quality markers and its impact on service delivery.
