Location: Hybrid, our office is at E1, Joseph’s Well, Leeds, LS3 1AB, working from home then site visits to third sector partners.

The Enhance Programme is a partnership between Leeds Older People’s Forum (LOPF), four third sector delivery partners, and Leeds Community Healthcare NHS Trust (LCH) services. Enhance links third-sector organisations with LCH Neighbourhood Teams and LCH specialist services to avoid delayed discharges and readmissions to hospitals for individuals. It supports people to attend Self Management Health Hubs as well as providing person-centred holistic support at home for people receiving community-based healthcare. Each delivery partner will be delivering a Self Management Health Hub in partnership with LCH. The project also aims to enhance capacity throughout the health and social care system.
Enhance is funded by Leeds Community Healthcare until 31st March 2026.

What’s involved?
We’re looking for an experienced project manager to:
1. Work with third sector organisations to deliver health hubs and wellbeing support to older people in receipt of community healthcare services.
2. Link with LCH Neighbourhood Teams and Business Units to ensure people 50+ who need support with health and wellbeing are referred to their relevant delivery partner.
3. Manage contracts of third sector partners to ensure their delivery meets the requirements of the Enhance contract.
This role will build on a substantial amount of project planning and implementation work which has already taken place. Third sector delivery partners, stakeholders, the Age Friendly Steering Group, and LOPF staff and Trustees, amongst others, have worked together since January 2022 to develop and establish the programme.
Skills and experience
As LOPF’s Enhance Project Manager, you will be well-organised and efficient, with excellent contract and relationship management skills. You’ll be experienced in partnership development and health sector funding. You will understand the issues affecting older people experiencing issues with their health, and the interventions that can be employed by third sector organisations.
