Join our Whitfield PCN Community Nursing Team as a Senior Community Nurse.
We currently have a 30 hours a week vacancy for a Senior Community Nurse in our Whitfield team based at Tunstall Primary Care Centre.
Community nursing teams deliver holistic, complex care to housebound patients within their own home environments. You will be responsible for leading the community nursing team, managing the caseload and supporting staff development. You will work alongside the leadership team and other senior nurses to ensure proactive and effective care delivery, liaising and working with the multidisciplinary team to ensure individualised and evidence based care to our patients.
Key Responsibilities:
* You will be responsible for allocating patient visits within the skill set for the community nursing team. You will work alongside the Lead Practitioner to manage the caseload, including reviewing demand and capacity.
* You will be competent in a variety of clinical skills, and be able to share your knowledge and expertise with others, signposting to more specialist services as appropriate.
* You will be an effective leader, demonstrating care, compassion and empathy and role modelling the Trust values. You will be able to support staff within the team to develop their own skills and will offer a high standard of supervision.
* You will be able to engage complex patients, agree mutually beneficial treatment plans and to plan, implement and evaluate care pathways.
* You will support with quality assurance, including audit, incident reporting and gaining patient feedback. You will work with the leadership team to evaluate this in line with objectives.

What We Ask From You:
* Working in a multi-disciplinary team alongside Clinical and Operational Leadership to ensure high quality care delivery to service users.
* Provide specialist assessment and care planning in conjunction with service users and carers.

Essential Qualifications and Training:
* Nursing Registration with the NMC.
* Degree in nursing (or equivalent).
* Mentorship/Assessor Qualification.
Essential Experience:
* Experience in MDT working.
* Community nursing experience.
* Evidence of providing clinical information and advice to others.
* Caseload management.
