Job summary We are looking for an enthusiastic Care Coordinator to work across the Sutton OP CMHT is based in North Cheam and makes up one of the 4 teams within the Sutton OP services. It is a multi disciplinary team comprised of Consultant Psychiatrists, Junior doctors, Team Manager, Nurses, OTs, Psychologists and RSWs. The team specialises in assessing and treating adults aged 75 and over with a mental health condition. Applicants need to be professionally qualified and committed to providing responsive and high-quality clinical services to mental health service users and their carers. Main duties of the job This role is for a Care Co-ordinator to work with patients with cognitive impairment and functional illnesses and so will need extensive relevant experience of working with Older People within a range of settings. Applicants must have a keen interest in all mental health issues affecting Older People and therefore is required to have up to date knowledge of treatments and protocols. The post holder will be able to work autonomously, exercise clear and coherent decision-making skills and be able to work flexibly to meet the needs of the wider service. The post sits alongside experienced clinicians of various disciplines, both within the CMHT as well as other sub teams. Applicants will need to have extensive relevant experience of working with Older People in a range of settings, ideally with some community experience but this is not essential. A keen interest in all mental health issues affecting Older People and up to date knowledge of treatments and protocols.
