In response to recent events nationally where people with a learning disability and autism have received less than optimal care we have developed our out of area and complex case manager role.
You will work with experts by experience and other clinicians to monitor and review those who are placed in out of area treatment facilities to ensure the care they receive is of high quality and for no longer than required. You will also work with system partners and very closely with our specialist social workers to ensure wherever possible hospital admissions are avoided and care and treatment is delivered within the home setting. This role is essential to support some of our most vulnerable and misunderstood clients to be seen and valued as citizens within their communities and not become the hidden., This role will be key to ensuring that those with the most complex needs are supported to access specialist mental health hospital care that is appropriate to their needs and is of high quality. You will also ensure that hospital admission will be for no longer than required and work with system partners to reduce likelihood of admissions and where this is not possible to ensure timely discharges
You will work autonomously to support and co-ordinate, where necessary, the case management of individuals with highly complex health and social care needs. You'll be working to support individuals and their families/carers in accessing the most appropriate care to meet their identified needs.
In this role you'll be ensuring the case management is delivered with the service user at the heart of the process and that case management issues are dealt with in line with best practice in a timely manner to achieve best outcomes for the individual. + Candidates not currently employed by the Trust who attend an interview for non-qualified Band 1-5 posts are required to undertake numeracy and literacy assessments.
