Lionwood Medical Practice is looking for an experienced Practice Nurse working within a primary care setting in the UK. The successful candidate will be responsible for treating patients with long term condition management, contraception checks, cervical screening, ear care, vaccination and immunisations as well as advising patients on their continuing nursing and medical needs.
The candidate will work according to practice protocols and guidelines, delivering care independently and in collaboration with the nurses and the wider practice team.
Main duties of the job
* Patient Consultations: Assess, plan, develop, implement and evaluate programmes to promote health and well-being for our patients. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
* Caseload Management: Manage long term condition care for patients autonomously, with support from the wider practice team.
* Care Planning: Develop ongoing health plans with patients and their families, emphasizing prevention and self-care, and following up on patient progress.
* Health Promotion: Support patients in adopting healthy lifestyles and self-care strategies. Including taking part in promotion campaigns within the practice.
* Collaboration: Work with the multidisciplinary team to ensure holistic and comprehensive patient care.
* Record Keeping: Maintain accurate, up-to-date patient records on the practices computer system (SystmOne).

Job responsibilities
* Chronic Disease Management and QOF Targets: Participate in chronic disease management and help the practice achieve its Quality and Outcomes Framework (QOF) targets.
* Clinical Audits and Protocol Development: Participate in practice clinical audits and contribute to the development of protocols and clinical guidelines for the practice. Ensure Practice safeguarding protocols are adhered to at all times, escalating incidences where appropriate. Feeding back and contributing to the clinical meetings to ensure the development.
Person Specification
Qualifications
* Registered Practice Nurse with current NMC registration. Recognised Practice Nurse qualification at degree level or equivalent.
Experience
* Knowledge of the needs of patients with long-term conditions.
* Awareness of clinical governance in primary care.
* Experience of cervical cytology, immunisation and vaccination, ear care, minor surgery and contraception checks.
* Change-management skills and ability to support patients to change lifestyle.
* Communication skills, both written and verbal.
* Ability to communicate difficult messages to patients and families.
* Experience of working in primary care.
* Experience of working in a GP practice.
