Are you a Qualified Mental Health Nurse, Social Worker or Occupational Therapist with urgent care experience looking to progress and lead on the development of our Home-Based Treatment Team (HBTT)?
An exciting opportunity has arisen within Salford HBTT as a 12-month fixed term role piloting a senior position in responding urgently to referrals from our Mental Health Liaison and Inpatient colleagues, offering alternatives to inpatient admission for people with complex presentations. The post holder would lead this initiative, seeing service users face to face and supporting HBTT staff members in building their confidence in working with complex presentations. The post holder would attend board rounds and patient flow meetings to understand service pressures and proactively support to reduce these within this role in the HBTT, advocating for people to recover at home in the least restrictive way. Additionally, there will be a requirement to supervise and support staff members within HBTT in their professional development.
Main duties of the job
The post holder will:
* Ensure a rapid response to all urgent referrals coming into the service, playing an integral role in supporting patient flow through the system.
* Support the service in developing and maintaining robust gatekeeping processes, incorporating the principles of person-centred care and least restrictive interventions.
* Proactively identify and facilitate supported early discharges from inpatient settings.
* Support admission avoidance from MHLS and community teams.
* Support team development to meet the Core Fidelity Standards.

Person specification
Qualifications
* Degree in RMN, Occupational Therapy or Social Work
* Additional qualification relevant to job role
Experience
* Experience in a leadership role
Skills
* Ability to manage complex cases autonomously
* Line management and staff development skills
Knowledge
* Understanding of the function and purpose of a Home Based Treatment Team
* Knowledge of the functions and purposes of our MHLT and inpatient colleagues
